Diabetes Telehealth Series
The Utah Diabetes Prevention and Control Program offers a monthly Diabetes Telehealth Series free of charge. It is held the 3rd Wednesday of each month from 12-1pm. This “Lunch Time” series has become very successful over the years and now averages over 100 participants each month. Starting with just a small group in Utah, participation has grown to include connections from Montana, Idaho, Wyoming, Colorado, Texas and most recently Louisiana.
To participate, complete this registration form.
Please register at least 2 days prior to the program date. This will help ensure availability of video conferencing sites. Contact the Diabetes Program office for late registrations and/or additional information.

CEU Credits
CEU credit is available: 1.5 hours for Nurses, 1 hour for Dietitians, and 1 hour for all others for the live archived presentations.
To receive CEU credit for the archived presentation, go to http://www.health.utah.gov/diabetes/telehealth/archives.htm to download the Power Point Presentation, evaluation form and post-test. Then, view the archived program and complete and return the evaluation and post-test. You must receive a post test score of 80% or better to qualify for CEU credits. CEU certificates will be emailed.
